Vaginal Infections and Types:
Introduction: Three common infections  are associated with vaginal discharge are

1 . Bacterial vaginosis

2. Trichomoniasis- Sexually Transmitted Disease(STD)

3.Vaginal Candidosis

Bacterial Vaginosis(BV):

It is characterised by an overgrowth of predominantly amaerobic organisms (e.g. Gardnerella vaginalis, Prevotella spp., Mycoplasma hominis, Mobiluncus spp.) in the vagina leading to a replacement of lactobacilli and an increase in vaginal pH. Bacterial Vaginosis (BV) is the commonest cause of abnormal vaginal discharge in women of child bearing age, but may also be encountered in menaopausal women, and is rather rear in children. In Asian women the prevalenceis less well studied, but in general around 20-30%. Women having sex with women share similar lactobacilliary types and are at increased risk for BV.

Pregnant women with Bacterial vaginosis are at higher risk of preterm delivery.

Trichomoniasis:

Trichomonas vaginalis (TV) is a flagellated prozoon which is a parasite of the genital tract. In adults it is almost exclusively sexually transmitted. Due to site specificity, infection only follows intravaginal or intraurethral inoculation of the organism.

Vaginal Candidosis:

Vulvovaginal Candidosis is caused by an overgrowth of Candida albicans in 90% of women (remainder other species eg C.glabrata). An estimated 75% of women will experience at least one episode during their lifetime. 10-20% women are asymptomatic vaginal carriers; this may be up to 40% during pregnancy.

Limitations of Antibiotics/ antimicrobials:

• > Provide symptomatic relief only
• > Cannot prevent recurrent infection
• > Kill natural flora also thereby increase chances of recurrent infections
• > Associated with lots of GI side effects like diarrhoea, C.difficile infection GI upset if taken orally
• > Higher chances of antibiotic resistance lead to ineffectiveness or decrease in the efficacy of antibiotics
• > Cannot restore normal vaginal ecosystem
  
> Cannot restore vaginal immunity

Role of Probiotic Therapy in the management of BV and Recurrent Vaginal infections:

The estimated number of microbial species inhabiting the vagina is 50, compared to approximately 800 species in the intestinal tract. The vaginal microflora of healthy premenopausal women is gener­ally dominated by Lactobacillus species.

Urogenital infections not caused by sexual transmission, namely yeast vaginitis, bacterial vaginosis, and urinary tract infection remain a major medical problem in terms of the number of women afflicted each year. Although antimicrobial therapy is generally effective at eradicating these infections, there is still a high incidence of recurrence. The patient’s quality of life is affected and many women become frustrated by the cycle of repeated antimicrobial agents whose effectiveness is diminishing due to increasing development of microbial resistance. There is good clinical evidence to show that the intestinal and urogenital microbial flora have a central role in maintaining both the health and wellbeing of humans. Furthermore, the use of “good bacteria” to replace or augment bacterial population is gradually achieving scientific acceptance. This application is termed probiotics: “live micro-organisms which when administered in adequate amounts confer a health benefit on the host”.  Probiotics generally used for vaginal infections are bacterial origin e.g. lactobacilli spp. belongs to vaginal flora.

Several clinical trials to date have been completed to study the effects of various strains of lactobacilli to colonize the vagina of women with symptomatic or as­ymptomatic BV, to reduce colonization of pathogens, and to improve symptoms and/or signs of BV when they are present.

In Conclusion, probiotics represents an excellent treatment for BV. Studies demonstrate that probiotics can reduce the risk of recurrence, restore the vaginal microflora, prevents the growth of pathogens, It is best alternative therapy compared to antimicrobials and reduces the risk of recurrence/relapse.

Probiotics are safe to use in pregnancy and lactating women and helpful to prevent the pregnancy & post IVF related complication like PROM, PTB, PID etc.

Prescribing Information

TrinZa is a combination of six species of Lactobacilli e.g. L. acidophilus, L.rhamnosus, L.reuteri, L.gasseri, L.fermentum, L.plantarum and Bifidobacterium longum. All the Lactobacilli are highly selective to the Uro-genital region and the part of Vaginal Microflora. These lactobacillus species are colonized the vagina and could inhibit the growth and the adhesion of urogenital pathogens.
